Enrollment Requirements For All Students:
1. Proof of Residency (one of the following)
A. Lease
B. Gas Bill (Nicor)
C. Electric Bill (Com Ed)
D. Water Bill
E. Closing Papers (if new purchase)
2. Birth Certificate
3. Social Security Number
If you are transferring from an Illinois Public School you need to provide the following:
1. State Transfer Form
2. Transcripts
3. Health Records
A. Physical Dated From Freshmen Year
B. All Immunizations
C. Proof of Graduation From 8th Grade (incoming freshmen only)
Freshmen & Sophomores are required to take placement tests
(approximate time - 3 hours)
Juniors & Seniors are required to take essential skills tests
(approximate time - 2 hours)
